Airfare:

  • Requirements:
    • Give at least 21 days’ notice before your travel date to request flight assistance.
    • Complete a medical clearance form:
      • The form must be filled out by the doctor who recently saw the patient.
      • We will fax this form to the doctor once your application is received.
    • Provide proof of income:
      • Show the total household income and assets for everyone living in the household.
      • Forms of proof include your complete income tax return for the current year and any award letters for Disability or SSI (if you receive them).
    • Mercy Medical Angels may sponsor a round-trip flight for the patient and one non-medical attendant (if the doctor says it’s needed) on a commercial airline, coach seating
  • The Patient Must:
    • Must be able to sit on their own once in the seat.
    • Require little or no medical attention during the flight.
    • Able to transfer from a wheelchair to the aircraft seat with little or no help (no one can lift them into the seat).
    • If oxygen is needed:
      • Some airlines may provide oxygen during the flight.
      • Otherwise, the patient must have an FAA-approved portable oxygen concentrator (POC).
      • Airlines need at least 48 hours’ notice to prepare for oxygen equipment.
      • The patient is responsible for arranging and paying any oxygen-related travel fees.
    • There is a minimal, nonrefundable ticket processing fee based on income, determined after verifying income documents.

Ground Transportation

  • Patients can receive gas cards or tickets for bus or Amtrak to get to medical treatment.
  • Trips must be for medical care located more than 50 miles away.
  • Gas cards are also available for multiple trips to the same treatment center if each trip is 100 miles or more.
